Immunizations

The amended School Immunization Law requires that a completed Certificate of Immunization form (CIS) be present at school on or before your child's first day of attendance. The CIS must show proof of full immunization, initiation of a schedule of immunization, or a certificate of exemption.

If a parent completes a Certificate of Exemption, and an outbreak of a vaccine-preventable disease for which the child is exempt occurs, the child may be excluded from school for the duration of the outbreak by order of the Public Health Officer.

You can download the Certificate of Immunization  (CIS) and the Certificate of Exemption (COE) forms on the Department of Health Website, or get a copy from your health care provider’s office or the health department.
