Administrative Supplemental Application Forms:
Administrative supplemental application forms are available in the "inserts" tab in the on-line program.
It is necessary that you have at least five Administrative Reference Forms completed and on file with the Human Services office. These evaluation forms are in addition to references or evaluations that might be in your placement file. The evaluation forms should be given to supervisors who are familiar with your most recent work.
The completed evaluation forms are confidential and should be mailed from the supervisors directly to the Snohomish School District.
