There are 3 parts to this survey.

1. Let us know what your plans are next year, this will serve as your Final Transcript Request.
2. Let us know if you are receiving any scholarships or academic awards. The Senior Award Night on June 1 honors student scholarships and academic awards. We only know about your scholarships through this survey! Please include athletic scholarships, institutional scholarships, WUE, and any other scholarships you have earned.
3. Tell us about your experience at GPHS
