Students planning on turning out for a sport need to have a sports physical completed by a physician before participating in any sports tryout or practice. All athletes must complete the online forms through Rank One for the current school year prior to tryouts/practice. All athletes must complete the online forms through the Rank One Sport website prior to tryouts/practice.
Forms can be found on the Rank One Sport website. Please note that the electronic forms on RankOne Sport need to be completed for your athlete each school year. Physicals are valid for 24 months provided they do not expire during the sports season. Current sports physical paperwork must be turned in to the Centennial Middle School Main Office in person to complete the process.
Athletes must purchase an ASB card, pay the athletic participation fee and have current medical insurance.
Please note: If an athlete is going to be picked up from a game/meet and will not be riding the bus back to CMS, they must check out with the coach at the event. If you are transporting another child, that parent must supply the coach with a note allowing you to transport their child.
Athletes must ride to an event with the team on the bus. Exceptions can only be approved by the Principal or Vice Principal. No bus transportation is provided back to Centennial Middle School for in-district meets/games. Students must be picked up at the event site (ie: meets/games at Snohomish High School, Glacier Peak High School, Bailey Farms or Valley View Middle School).
