Employee Leave of Absence

The district understands that employees will need time away from work for many different types of situations.

Leave of absences include:

  • Serious health conditions for yourself or a family member
  • Pregnancy-related disability (prior to baby's birth)
  • Maternity/paternity/baby bonding
  • Military/military caregiver leave
  • Student teacher
  • Job share
  • Unpaid long-term leave (See collective bargaining agreement (CBA) for details/eligibility, e.g. professional improvement.)
  • Unpaid short-term leave (five workdays or less)

Medical Leave of Absence Request Instructions

Contact the Leave Specialist

  • Contact the Payroll Department Leave Specialist for guidance and to initiate your leave request. The Leave Specialist will assist throughout the process, help answer questions and ensure understanding of the necessary steps. 
  • You will be provided the district's Medical Certification From. This should be submitted to your physician promptly to avoid delays in processing. 

Submit Leave Request Form via Droplet Forms

  • Fill out the district Leave of Absence Request Form. The form can be found:
  • Submission steps:
    • Logging in:
      • Enter your school district email address and click "continue."
      • First-time users: Review and accept Droplet's terms of service.
    • Access the form:
      • Upon login, you'll land on your Droplet dashboard. Select "start new form" or proceed directly if using a specific form link.
    • Complete the form:
      • Fill out all required fields.
      • You have the option to attach the Medical Certification Form completed by your physician. You may also return the Medical Certification Form to the Leave Specialist for review and processing.
    • Submit:
      • Review for accuracy.
      • Click "submit" to send your request to Human Resource Services for processing.

Important Notes

  • Timing- Submit your request at least 30 days in advance for foreseeable leaves (e.g., planned surgery). For emergencies, notify the Leave Specialist as soon as practicable. 
  • Support- Contact the Leave Specialist for assistance with Droplet or documentation.
